How do you get the icon in the browser bar

OK, you all know that nice icon up at the top of the page in the bar just before hhtp:/ and loves to reside in the favourites, yep the one that looks like a tick on a mauve background up there as you are reading this.

How do you customise it for your own web site?

Google fav.icon and you'll get several hits on how to create these. The problem is they do not last long and disappear after a few PC restarts.

One strange thing is my icons will hang in favorites for a while and then revert to the blue 'e'.
