Leviton VRF01-1LZ Z-Wave Fan Control - sudden buzzing

The switch is about 6 years old, and has been installed in a couple other locations. (about 2 1/2 years in current apartment)

a couple days ago, the fan stated making noise at low levels.
seems to go quiet at 67% and above.

and the really weird part... when i turn the switch off. then the fan buzzes rhythmically, and the speed of the buzz changes depending on how fast the blades are still turning.
the fan never completely stops.
If i manually stop it, the buzz becomes solid, and the fan will re-start once i let go.

if i do the manual over-ride where you pop the switch to fully disconnect, the fan stops.


My guess is something has failed in the switch, and i need a new one.
unless someone has a suggestion otherwise.

I might check later to make sure the neutral didn't come loose behind the switch, but i haven't been in there since i installed 2 years ago.

so now.... going on a couple more days, it buzzes at all speeds, and runs slowly.

at least it did before it found it's way to the trash.

now to live with an old switch for a month or 2. hopefully the homeseer fan switch will be out by the time my soon to start renovations are done in a couple months.
