Maybe an errant/misconfigured direct association from another device/switch? It would fit the "not in events, not reflected by HS controls".

Ghost of Christmas Past paying a visit?
One of 3 GE/Jasco m/n 14287 z-wave fan controllers has mysteriously come on at high speed twice in the past few days. It's in my office and I was present both times, so I know no one pressed the button. Interestingly, it also isn't reporting the change to HS4; the HS device continues to show the fan as "off". The switch is fully controllable from HS, so no problems with z-wave comms. And yes, I've checked for forgotten events. It's used in two: one "all off" that runs when the house is vacated (hasn't run since yesterday) and one that automatically turns the fan off after a set time period (hasn't run since last month). Weird. Maybe an odd hardware failure mode? Any other ideas?Tags: None
