What is it from and how do I correct it?

There's a z-wave.db file in the Data\Z-Wave folder. I'm guessing that it got corrupted somehow. Shut down the z-wave plugin, either restore the file from a backup or rename that file, and then restart the plugin. If you rename it, the plugin will recreate the file.
