The error says it cannot open the database file when InitIO is called from HS3, so I'd start there. Obviously it's "there" since you can open it with the SQL browser, so perhaps it's the permissions....

Try rescanning it from the Root Thermostat Z-wave tab. That works for the Trane Thermostats. Unfortunately, there's no programmatic way to rescan (at least I can find) so you have to manually do it...
