I see one HS-WS100+ switch doing this constantly

Any ideas why this is happening with this switch?
    Maybe it is bad
It does it when it is on or off

I wouldn't suspect dirty contacts on a switch this new, but it does look like it is malfunctioning.

I would try each of these in this order, testing after each to see if the problem is cured
      1. Power cycle the switch
      2. rescan and optimize
      3. exclude/include again
      4. get HS to RMA the switch

Try power cycling the switch, maybe its in a bad state. If that does not help, try removing and re-adding as that will reset it. You could also try updating the firmware to latest in the loaded firmware is bad. If this all fails, we can replace it. I have never seen this issue before.
