      Here's a screenshot to remove/disable a code and its similar for adding/enabling a code (you need to enter the code in that action).

              HS3 Pro Edition 3.0.0.300 (Windows)
              Z-wave 3.0.1.94

              The Z-wave interface goes offline and I have to disable/re-enable to get it working.

                I would shut down HS, go to the \Data\Z-Wave folder and rename zwave2.db to something else, restart and see if that resolves it.

                        I have restarted. If I use the zwave tab to enter a user code it works. Here is the Advanced tab:

                          The SubType in the Device Type Internal value should be Entry Control, but yours is blank. You may need to bring the lock right next to your controller and rescan that way. If that does not work, you may need to exclude and then re-include the lock, which also would need to be done with the lock very close.
